You work at a health care facility (General Hospital) that has experienced an increase in patient falls on the Med/Surg ward over the last year. Put together a PI team and create the following:

• List of team members, their work area, and why they have been selected for the team (How are they a part of this issue?).
• Write a concise Mission Statement and Vision Statement for your team - These should relate directly to the purpose of the team (mission) and what they hope to accomplish (vision). No fluff or broad facility wide statements!
• Design a tool to measure/identify the problem (do NOT use patient satisfaction surveys)
  • for example - a check list for types/locations of falls
  • a chart showing the number of falls over a period of time (30 days).
• Design a chart or graph to display your findings
• List some possible solutions
• Create a powerpoint presentation (8-10 slides) of this entire process to present to your very dignified CEO (Me).
• Keep in mind, you will have to make up much of this information, however, using your creative skills, you can display a very realistic and effective use of a PI team

Patient Fall is a major patient safety issue which is preventable most of the time .

The PI team to investigate Fall would include the following members

Floor nursing Incharges

Floor housekeeping supervisor

Nurse Manager for quality and safety

House keeping executive

Nurse Manager staff development

Quality Officer

Nursing head

The team consist of interpersonal personnel's because falls have any aspects the causes of fall cam be varied between a physiological fall to environmental fall so a mix of nursing and housekeeping personnel's are needed to assess the patients risk for fall and to keep a safe environment
