As a result of a HIPAA performance improvement initiative, you have a been asked to create an employee review checklist or form to support the employee review process. The purpose of the initiative is to increase employee awareness. Your document should be directly related to how employee activities or training can contribute to the performance improvement initiative.

A performance improvement initiative is developed in order to make employees aware of their performance so that they can observe their mistakes and improve on those area that need improvement.

Quantifying employee performance is required in order to rate their performance.

The various criterias which can be used to rate employee performance are as follows:

  1. Productivity
  2. Quality
  3. Initiatives
  4. Schedule Adherence
  5. Policy compliance

The above five criteria has to be rated based on attributes constituting them thereby helping us to evaluate the same.

The total score of the employee has to be computed for 500 and based on scales their performance has to be coined as

  1. Exceptional - above 450
  2. Exceeds expectations - 400 - 449
  3. Meets expectations - 350 - 399
  4. Needs improvement - 275 - 349
  5. Poor performance - less than 275

This will help us to assess employee performance and thereby evaluate their performance.
