Answer - Interprofessional health care team includes physicians, nurses, social workers, chaplains, volunteers, home health aides, bereavement counsellors, dieticians and pharmacist.They all play a specific role and provide palliative and hospice care.
Their role are such as following -
Physicians - He play most important role in hospice care, he discuss hospice care with individual who is in terminal illness. He prescribe best medicines which provide comfort, painless effect and supervise care.
Nurse - A hospice nurse engage herself in entire care on end of life care. She mange pain symptoms and all necessary care which according to by physician's prescription. She provide psychological support to hospice patients and their family.
Social worker - Hospice social workers's important responsibilities include conducting psychosocial assessments, coordinating care, providing counseling and psychotherapy, intervening in client crisis situations, and educating patients and families about their treatment plan and the resources and support systems that are available to them.
Dietitians - Dietitians also find themselves in the role of providing psychological support to patients and families, particularly given the social significance of eating and meals, and providing nutritional counselling.
Pharmacist - He play an active role in hospice and palliative care through a wide variety of approaches, including medication order assessments and counseling the hospice team.
Chaplines - He provides hospice team is to care for patients and their loved ones: mind, body and spirit. While the chaplain focuses on the spiritual side.
