Description of skill-
Surgical Asepsis - It is the absence of all microorganisms within any type of invasive procedure. It involves the elimination of both pathogenic and non-pathogenic microorganisms. For example, a catheter is inserted into the patient's bladder using surgical asepsis.
Indications -
Prevention of healthcare-associated infections. These commonly include:
1) Catheter-associated UTI
2) Central line-associated bloodstream infection
3) Surgical site infection
4) Ventilator-associated pneumonia
5) Clostridium difficile infection
Outcomes/Evaluation-
The evaluation aims to evaluate the infection control plan or to identify the issue that needs attention. The elements of the infection chain are recognized and the measures are taken to prevent the onset of infection.
Considerations-
Nursing Interventions (pre,intra,post)-
1) A sterile object remains sterile only when touched by another sterile object.
2) Only sterile objects may be placed on a sterile field.
3) A sterile object or field out of vision or an object held below the waist is contaminated.
4) A sterile object or field becomes contaminated by prolonged exposure to air.
5) When a sterile surface comes in contact with a wet, contaminated surface, the sterile object or field becomes contaminated.
6) Fluids flow in the direction of gravity.
7) The edge of the sterile field or container is considered contaminated.
8) If the integrity of the sterile package is questionable, the item should not be used.
Client Education-
1) Sterile persons or sterile objects may only contact sterile areas; non-sterile persons or items contact only non- sterile areas.
2) Movement around and in the sterile field must not compromise or contaminate the sterile field.
3) Do not sneeze, cough, laugh, or talk over the sterile field.
4) Maintain a safe space or margin of safety between sterile and non-sterile objects and areas.
5) Refrain from reaching over the sterile field.
6) Keep operating room (OR) traffic to a minimum, and keep doors closed.
7) Keep hair tied back.
8) When pouring sterile solutions, only the lip and inner cap of the pouring container is considered sterile. The pouring container must not touch any part of the sterile field. Avoid splashes.
Potential Complications-
1) Catheter-associated UTI.
2) Central line-associated blood infections.
3) Surgical site infections.
Potential complications - Nursing Intervention
1) The nurse handle and prepare operating instruments and equipment within the sterile field.
2) Cleaning in the OR during the operation aims to remove the adherent visible soil, such as blood, tissue, and pus, from the surfaces, crevices, and jaws of instruments, devices, and equipment usually with cleaning agents and water.
3) Effective communication.
