increased blood glucose level is the stimulus, sensors sense the presence high blood glucose level and then beta-cells in the Islands of Langerhans secrete insulin, this is the response, then it acts on effectors and stimulates to take glucose into the cell.

In the given situation the response till the insulin secretion is normal, but the cells ( effectors ) are not taking the glucose into the cell so the effectors in the feedback system is affected so the answer is c) the effector.
