Thinking about the organotherapy fad of the early 1900s, one reason it did not work was that patients were injected with a water-based extract. Why is this a problem for testosterone?

Testosterone is a steroid hormone synthesized from cholesterol. The four rings of cholesterol A, B, C, D are present in testosterone. A, B and C are six carbon rings while D is a five-carbon ring. These rings make testosterone a zing zag structure that is insoluble in water. This testosterone is a non-polar molecule. Testosterone also has to pass through the cell membrane, which has a hydrophobic core. If testosterone is added to water, it will not dissolve in water due to its non-polar nature. The body flushes out this hormone in urine by first modifying its structure. Hence, water based extract did not work for testosterone as the testosterone could not be dissolved/extracted in water.
