As per the requirement listed, only question 3 is expected to be answered.
When the Senior Engineer challenged the basic performance assumption, he was told that the problem was outside the scope of responsibilities. There seems to be a situation where no regards were paid to a reality that when problem occur, they do not occur and is placed on plate of the attendant as per their scope of responsibility.
Thus, when error were to occur in future, it would be a error of omission.
This is because, people are aware that such error might occur in future with some probability. Even Senior Engineer pointed flaw in the basic assumption. Still, when the trainer insisted on accepting the assumption as a known fact, one is actually ignoring the very possibility of occurrence of such error.
No remedial action plan is put in place to deal with, when such error occurs.Thus, this is an issue of error of omission.
