HIS is a combination of information,technology,people and processes which interact together to collect,process and store information to support the business of the organization. Health information system is a technology which supports health care functions.it includes all computer systems,networks and the data those systems create and capture through the use of software.

The current erratic and inconsistent use of paper records and computers to document ,store and retrieve patient care information is undergoing a major upheavel as federal initiatives promote the development of a uniform electronic health record.Patient records in the VAs(Department of Veterans affair)  electronic health system are fully electronic ,portable and readily accessi ble .The VA developed in theEHR system to provide a single place for the health care providers to review and update a patients health record and to order medications ,special procedures,x-rays,nursing orders and diets.All aspects of patient records are integrated ,including active problems ,allergies ,current medications,lab results,vital signs and outpatient clinic history.

A national uniform EHR similar .to the VAs health record has the potential of greatly reducing the medical error associated with traditional paper records and vsatly improving patient safety and quality of care.it will allow all providers to more rapidly access patient speciic information and hopefully decrease the cost of health care.The goal of the government for EHR is that uniform standards will be adopted throughout the health care industry that will lead to a national EHR for most Americans within the foreseable future.

Part of a uniform EHR includes the use of a comprehensive ,standardized medical vocabulary.One such standardized vocabulary is the Systematized Nomenclature of Medicine Clinical (SNOWMED CT).The uses of SNOWMED CT include electronic medical records,intensive care monitoring,clinical decision support,medical research studies ,clinical trials and consumer health informatics.Some current computer system allows nurses to enter assessment data and interventions and record treatment and medication administration.Each piece of data that gets entered into the record can be tracked and reported on for many purposes .When nursing terminologies are used in information systems for documentation of nursing practice ,nurses can track and report on the benefits of nursing care.This would serve not only to improve practice guidelines,but also to facilitate nursing research and easily demonstrate the effectiveness of nursing interventions.This will make nursing care visible while providing a continuing evaluation of nursings efficacy.
