One of the ways a nurse uses guided research in a clinical setting is through the 5 minute head to toe assessment. Evidence based practice has shown that if a patient is stable that this assessment will reveal no abnormalities. If there is something wrong with a patient then evidence of the issue will most likely appear in the head to toe assessment due to the fact that most organ systems are covered during this assessment. If a patient is experiencing distress or is unstable it will be evident in either the vital signs, LOC, or be revealed during the assessment at some point. It is smart to follow evidence based practice to ensure that the same mistakes are not made again in the future. Evidence based practice comes from clinical trial and error and it just does not make sense to make the same mistakes twice and jeopardize other patients when we can prevent such events.

Evidence-based practice(EBP) used for patient care to make current decisions with best practice. Effective clinical decision making and intervention need EBP. Nurses special knowledge and skills needed in various clinical setup to apply EBP. Knowledge and skills development should be improved in the graduation and learning period for their best practice. Strategies can help incorporate with nurse self-assessment, personal approach to providing EBP care. In critical care unit Evidence-based practice used as a problem solving and scientific approach to avoid harm like avoiding DVT, drug overdose, Fall, etc.
