To improve a patient's overall health behaviors and outcome the healthcare and staff need to plan out specific goals and interventions that fit the patient's needs, taking into consideration the patient's culture, support system, mental health and physical health. This goal must be individualized to the patient, allowing for positive rapport between patient and the provider. The first element in motivating a patient to improve health behaviors is developing a positive relationship with the patient, to develop this positive relationship clinicians must build a trusting relationship through open communication. When the provider involves the family in the plan of care of a patient it can improve cooperation in a patient because this is where health behaviors are developed, maintained and changed (Garcia-Huidobro & Mendenhall,2015).
The post describes the way to improve patient's overall health.This is possible by providing a care to each specific patient in relation to their culture, support system, mental and physical health by the healthcare professionals. When the goals are individualized it creates a positive relationships or optimism, trust and improvised communication between a provider and patient. Along with this when the family are involved the behaviour of patient can be changed and modified for a better health outcome
