Conduct a health history on a family member or friend. Be sure they give you permission. Using the interviewing techniques learned in Module 2, gather the following information. Use your textbook as your guide. Present Health Past Health Family History While this is only a partial health history, summarize in 3 -5 pages the information you gathered. Also, answer the following questions: Was the person willing to share the information? If they were not, what did you do to encourage them? Was there any part of the interview that was more challenging? If so, what part and how did you deal with it? How comfortable were you taking a health history? What interviewing techniques did you use? Were there any that were difficult and if so, how did you overcome the difficulty? Now that you have taken a health history discuss how this information can assist the nurse in determining the health status of a client.

HEALTH HISTORY

Ms M

Age- 54years

Ht- 154 cm

wt- 75kg (overweight)

PRESENT

Ms S is presently having pain in the heels on exertion and is taking calcium pills for the same. X-ray revealed heel spur as the reason for heel pain

She is on hypertensive and diabetic medication both are under control with medication.

PAST

Ms S is on hypertensive and diabetic medications from last 10 years

She has been operated for laser eye surgery for correction of refractive error 7 years back

She has been diagnosed for having osteoporosis 5 years back

She has recurrent vaginal infections with itching and white discharge. Especially when not able to control her diabetes.

FAMILY

Her mother was a diabetic patient

Her father expired in an accident at the age of 35 with no known medical or surgical history.

She has three children, delivered through a normal delivery.

QUESTIONS

1) Yes, she was willing to share the information, i encouraged her to give detail information for being a active listener and putting up open ended questions whevever necessary. I also assured that the information will not be disclosed.

2) whole collecting history about vaginal infections patient was not willing to share anything.

I made her feel comfortable be talking of other patients with same problem

3) i was very relaxed while aking health history

I made the patient feel very free and friendly. Selected a casual surrounding with no disturbance and encouraged active participation of patient.

4) collecting history about sexual health was a bit challenging, bit i overcomed it by maintaining a healthy talk and gaining the trust of the patient, creating a friendly environment.

5) According to the collected health history patient is at a risk for cardiovascular diseases as she is overweight as well as a diabetic and hypertensive patient.

As she is having osteoporosis, she is at a risk for fractures.

Recurrent vaginal infections suggest poor diabetic control.
