Conduct a health history on a family member or friend. Be sure they give you permission. Using the interviewing techniques learned in Module 2, gather the following information. Use your textbook as your guide.

Family History

While this is only a partial health history, summarize in 3 -5 pages the information you gathered. Also, answer the following questions:

Was the person willing to share the information? If they were not, what did you do to encourage them?

Was there any part of the interview that was more challenging? If so, what part and how did you deal with it?

How comfortable were you taking a health history?

What interviewing techniques did you use? Were there any that were difficult and if so, how did you overcome the difficulty?

Now that you have taken a health history discuss how this information can assist the nurse in determining the health status of a client.

Conducting an health history of a family friend: Diagnose for ovarian cancer stage c Initial symptoms: Bloating stomach, appetite loss, weight loss, mild pain in low er abdomen area. family history: Menopause had two years before, at the time of diagnosis she is 47 years old. Few members of family relation were diagnose with cancer in past but where not studied properly the type of their diagnosis Initial screening: Went on screening of blood test, ESR level was high, ultrasound etc. were done Deep diagnosis: Ca125 marker were tested and the value observed was 3000+ (Normal range of Ca125 marker is 35), pet scan diagnose 3rd stage cancer Treatment planning: Six cycles of chemotherapy was given in order to lower down the metastatic speed and surgical removal of uterus and ovaries were planned after six months. After surgeries patients was monitored and level of ca125 and CBC was checked repeatedly. Platelet count drop drastically, multiple blood transfusion was given at each stage of chemo cycle and post operation Future plan: Monitoring of ca 125 level after every two months, every three months after pet scan and other biochemical test should be conducted. In case metastatic is seen new regimen of drugs can be planned in order to control recurrence of cancer Diets: Shifting to vegetarian diets, eating lots of herbs, carrot juice, fruits, maintaining yoga and meditation regularly in order to keep our immune system in check regularly She is my close relative. From the day she is diagnosed with cancer, we helped her in thinking positively about the entire treatment so that she can face and deal with the entire treatment positively. Dealing with those days were very difficult when she use to feel low and feel scared to lose hope due to fluctuating parameters observed in test results. I helped her in deviating her thoughts, use to encourage her by telling her how she is important to her family and how much hard work her own husband is doing to keep her in life. Talking to her use to give even me immense courage and strength. She is mentally strong lady and that helped her in every cycle of treatment Currently they understood cancer is a part of their present and they have to deal together with it The family is together and united in such time and love each other more. I being a medical profession person just helped them w ith right clinician and not to get panic at the times when test results were not that great but they showed immense level of courage and strength. It was real life pleasure and satisfaction to be part of their life during such difficult times.
