SYSTEM DISORDER

Diagnosis : Stomatitis.(Inflammation of mucous membrane of mouth and lips).

Pathophysiology

         * Number of factors like malnutrition,allergens or irritants,fungal infections

           radiotherapy,chemotherapy and immune mediated responses result in

           alterations in the mucous membarine of the mouth and lips.

         * This type of alterations result in inflammatory changes in the mouth and

        lips and leads to ulcerations in the oral cavity.

Health promotion and disease prevention.

         * Maintain a normal dietry pattern which include vegetables,fruits to avoid

           vitamin B and iron deficiency stomatitis.

         * Avoid biting of lips and contact with irritant substances.

         * Maintain good oral hygeine to avoid oral infections.

ASSESSMENT.

    Risk factors.

         * Allergic foods and chemicals.

         * Malnutriton.

         * Compromised immune status.

         * Autoimmune diseases.

         * Fungal infections (Candidasis).

         * Presence of dentures and orthodontic bands.

     Expected findings.

         * Pain in the mouth.

         * Presence of mouth ulcers.

         * Swelling of mouth.

         * Blisters and redness present on the mouth.

         * Difficulty to speech,eat and sleep due to mouth ulcer.

    Laboratory tests.

         * Blood CBC count (complete blood cell).

         * Culture of samples taken from the lesions.

         * Tissue biopsy of samples taken from the lesion.

   Diagnostic procedures.

         * Diagnosis of stomatitis can done through

                   * Physical examination.

                   * Biopsy of ulcers or lesions.

                   * Culture of lesions.

PATIENT CENTERED CARE.

* Nursing care.

* Prepare a diet plan for the patient to maintain normal

nutritional pattern.

* Provide psychological support to reduce the anxiety level.

              * Instruct the patient to maintain oral hygiene to avoid infections

                and enhance recovery process.

              * Application of coating agents.(eg; sucralfate )

              * Application of topical analgesics (eg; Benzydamine).

              * Application of topical anesthetics (eg; lidocaine.)

              * Anesthetics.(eg ; Benzocaine.)

              * Topical analgesics.( eg; Benzydamine hydrochloride)

              * Steroids.(eg; prednesolone)

              * Antifungal drugs.(eg ; clotrimazole)

              * Antiviral drugs. (eg ; Aciclovir)

         Client education.

                * Educate about importance of balanced diet to avoid nutrition

                   deficiency stomatitis.

                * Educate about importance of oral hygiene to prevent oral infection.

                * Instruct to avoid contact with irritant chemicals and allergic foods.

         Interprofessional care.

               * Maintain the oral hygiene of stomatitis patient by using mouthwash.

               * Provide a peaceful environment to the patient.

               * Give psychological support.

               * Maintain the nutritional status of the patient.

                Give vitamin supplements to avoid vitamin deficiency stomatitis.

        Safety considerations.

               * Place the patient away from allergens and irritant substances.

               * Give mouth washes to maintain oral hygiene.

               * Maintain aseptic techniques during nursing care.

        Complications.

              * Bleeding from the mouth.

              * Mouth infections progress to systemic diseases.
