“According to the Institute of Medicine (IOM) report, To Err Is Human,4 the majority of medical errors result from faulty systems and processes, not individuals”(Hughes, 2008).Because errors are caused by some fault in the system it is important to implement several methods to identify inadequacies in the care patients receive as well as ways to prevent future errors from occurring. Many hospitals develop and implement important protocols to increase quality improvement and better the care patients receive while in the inpatient setting. One example includes purposeful hourly rounding in which healthcare providers are to check on their patients every hour and address the any issues/changes in the patients’ health status. The goals of measuring health care quality are to determine the effects of health care on desired outcomes and to assess that it is consistent with consistent with patient likings.

The error that are occurred in the hospital setting is due to the faulty system and also due to the individuals. Human should not be error less in every time. Individual may make an error while performing care to the client, receiving data from the patient, recording the patient values and findings. Faulty system and the process is also one of the major reason that errors are occurred in the health care setting.
